The Luzerne County Assistance Office frequently requests that their clients referred to the ALTA Program complete Attendance and Participation forms to document attendance within the Adult Learners’ Training and Assistance Program. Please request the ALTA Program adult learner to bring the form to each ALTA session so that the ALTA instructor can verify with his/her initials that the client has attended. It is the client’s responsibility to bring the form to each class. At the end of the month, please obtain the L.C.A.O. participation and attendance form and place the original on my desk. The L.C.A.O. participation and attendance form will be copied; the copy will be filed in the client’s confidential folder and the original will be returned to the Luzerne County Assistance Office.
